Atuação da psicologia na UTI frente ao paciente em desmame ventilatório

Abstract

A inserção do psicólogo nas Unidades de Terapia Intensiva (UTI) reflete a necessidade de práticas mais humanizadas no contexto hospitalar. A UTI é um meio potencialmente iatrogênico onde o paciente está sujeito a diversas situações estressoras, dentre elas, destaca-se o processo de desmame ventilatório, cujo êxito está intrinsecamente relacionado aos aspectos psicológicos. Nesse cenário, o presente estudo buscou investigar esse fenômeno com o objetivo de construir instrumentos que subsidiem o desenvolvimento de práticas e intervenções psicológicas direcionadas aos pacientes em desmame ventilatório. Com essa finalidade, foram realizadas entrevistas semi-estruturadas com profissionais e estagiários de psicologia em atuação na Unidade de Terapia Intensiva do Hospital Geral de Vitória da Conquista. O estudo apresenta delineamento qualitativo de caráter exploratório-descritivo. Para a interpretação dos dados, foi utilizada a análise de conteúdo, método proposto por Bardin. O procedimento possibilitou a identificação das seguintes categorias: 1. Aspectos emocionais e orgânicos dos pacientes em desmame ventilatório; 2. Estratégias interventivas do psicólogo frente ao paciente em desmame ventilatório; 3. Atuação do psicólogo junto à equipe multiprofissional; e 4. Atuação do psicólogo frente à família do paciente. O processo de categorização proporcionou a construção de um protocolo de intervenção e um fluxograma direcionado ao atendimento psicológico de pacientes em desmame ventilatório. Conclui-se que os profissionais de psicologia hospitalar utilizam uma série de técnicas teoricamente embasadas frente ao paciente em desmame ventilatório, sendo viável a operacionalização e sistematização dessas técnicas por meio da utilização de protocolos e fluxogramas.

The insertion of the psychologist in the Intensive Care Units reflects the need for more humanized practices in the hospital context. The ICU is a potentially iatrogenic environment where the patient is subject to several stressful situations, among them, the ventilatory weaning process, whose success is intrinsically related to the psychological aspects, stands out. In this scenario, the present study sought to investigate this phenomenon with the objective of constructing instruments that subsidize the development of practices and psychological interventions directed to patients in ventilatory weaning. For this purpose, semi-structured interviews were conducted with professionals and psychology interns working in the Intensive Care Unit of the General Hospital of Vitória da Conquista. The study presents a qualitative exploratory - descriptive character. To analyze the data, we used the content analysis method proposed by Bardin. This method allowed the identification of the following categories: 1. Emotional and organic aspects of patients in ventilatory weaning; 2. Intervention strategies of the psychologist regarding the patient in ventilatory weaning; 3. Psychologist's performance with the multiprofessional team; and 4. Psychologist's performance in front of the patient's family. This categorization provided the construction of an intervention protocol and a flow chart directed to the psychological care of patients in ventilatory weaning. It is concluded that the hospital psychology professionals use a series of techniques theoretically based on the patient in ventilatory weaning, being feasible the operationalization and systematization of these techniques through the use of protocols and flowcharts.
