dc.description.abstractIntroduction – During the COVID-19 pandemic, patients with acute appendicitis (AA) found the health system overloaded and, in many centers, it was difficult to care for patients with the virus and abdominal emergencies, with some difficulty in accessing a hospital bed and possible clinical consequences. Objective – To compare the frequency, severity and outcomes of acute appendicitis in a public referral hospital for emergency abdominal surgery in the city of Salvador, Bahia, in the trans-pandemic period of COVID-19 with the pre-pandemic period. Methodology – Inspired by an ecological study, using biological and patient profile variables, the retrospective study comprised two series with all patients undergoing appendectomy at the Salvador Municipal Hospital (HMS) from March 1, 2019 thru February 28, 2020 (pre-pandemic period) and from March 1, 2020 thru February 28, 2021 (first transpandemic year). Severity variables (time of symptom evolution, AA classification by severity and anesthetic risk profile), types of surgery and outcomes (hospital stay, complications and mortality) were studied for comparative analysis. Results – A total of 882 patients underwent appendectomies due to AA were studied, with 412 found in the transpandemic series (2020) and 470 appendectomies in the previous year (2019). The average time from AA symptoms to surgical intervention during the pandemic was 4.2 days (median = 3.0 days) versus 3.5 days (median = 2.0 days) in the period without the pandemic. Rates of acute perforated appendicitis reached 33.4% in 2020 (during the pandemic) against 27.3% in 2019. During the pandemic, 20 (twenty) appendectomies were included for acute appendicitis in the presence of symptoms and/or positive serology for COVID -19, which resulted in four deaths and a mortality rate of 20% in this association. During the pandemic, the mortality rate was approximately 3 times higher (1.7%), when compared to the mortality of appendectomy patients during the pandemic (0.6%). The length of hospital stay was an average of 4.4 days during the transpandemic, while it was 3.4 days pre-pandemic. Laparoscopic appendectomy was the most used technique pre-pandemic (65.3%) while during the trans-pandemic open appendectomies predominated (55.1%). Postoperative complications in the pandemic series were 29.3%, compared to 23.8% before the pandemic. Reoperations were necessary in 10.7% during the pandemic period compared to 5.5% in 2019. The rate of negative appendectomies was maintained in both periods at 4.2 and 4.4%, respectively. Conclusion – The impact of the COVID-19 pandemic produced significant changes in the time of evolution of symptoms, diagnosis and surgical treatment of AA, related to greater severity, rate of reoperations, length of hospital stays and postoperative complications.pt_BR

dc.description.resumoIntrodução – No período da pandemia de COVID-19, pacientes com apendicite aguda (AA) encontraram o sistema de saúde sobrecarregado e, em muitos centros, houve dificuldade de atender pacientes com a virose e com urgências abdominais, com alguma dificuldade de acesso ao leito hospitalar e possíveis reflexos clínicos. Objetivo – Comparar a frequência, a gravidade e os desfechos da apendicite aguda em hospital público de referência para cirurgia abdominal de urgência, na cidade de Salvador, Bahia, no período transpandemia de COVID-19 e no período de pré-pandemia. Metodologia – Inspirada em estudo ecológico, utilizando variáveis biológicas e de perfil do paciente, a investigação retrospectiva compreendeu duas séries, com todos os pacientes submetidos à apendicectomia no Hospital Municipal de Salvador (HMS), entre 1º de março de 2019 e 28 de fevereiro de 2020 (período pré-pandemia) e de 1º de março de 2020 até 28 de feverreiro de 2021 (primeiro ano transpandemia). Foram estudadas variáveis de gravidade (tempo de evolução de sintomas, classificação da AA por gravidade e perfil de risco anestésico), tipos de cirurgia e desfechos (permanência hospitalar, complicações e mortalidade) para análise comparativa. Resultados – Foram estudados, no total, 882 pacientes submetidos a apendicectomias por AA, sendo encontrados 412 na série transpandemia (2020) e 470 apendicectomia no ano anterior (2019). O tempo médio de sintomas de AA até a intervenção cirurgica, durante a pandemia, foi de 4,2 dias (mediana = 3,0 dias), contra 3,5 dias (mediana = 2,0 dias) no período sem a pandemia. As taxas de apendicite aguda perfurada chegaram a 33,4% em 2020 (durante a pandemia), contra 27,3% em 2019. Na transpandemia, foram incluídos 20 (vinte) apendicectomias para apendicite aguda na vigência de sintomas e (ou) sorologia positiva para COVID-19, que resultaram em quatro óbitos e taxa de mortalidade de 20% nessa associação. Na pandemia, a taxa de mortalidade foi cerca de três vezes maior (1,7%), quando comparada à mortalidade dos apendicectomizados durante a pandemia (de 0,6%). O tempo de permanência hospitalar foi, em média, de 4,4 dias na transpandemia, enquanto, na pré-pandemia, foi de 3,4 dias. A apendicectomia laparoscópica era a técnica mais usada na pré-pandemia (65,3%), enquanto, na transpandemia, predominaram as apendicectomias abertas (55,1%). As complicações pós-operatórias na série da pandemia foi de 29,3%, contra 23,8% antes da pandemia. As reoperações foram necessárias em 10,7% no período da pandemia, contra 5,5% em 2019. A taxa de apendicectomias negativas esteve mantida, em ambos os períodos, em 4,2 e 4,4%, respectivamente. Conclusão – O impacto da pandemia de COVID-19 produziu mudanças significativas no tempo de evolução de sintomas, diagnóstico e tratamento cirúrgico da AA, relacionadas com maior gravidade, taxa de reoperações, tempo de permanência hospitalar e complicações pós-operatórias.pt_BR
