dc.description.abstractChronic Rhinosinusitis with Nasal Polyps (CRSwNP) results from degeneration of the sinonasal mucosa. The prevalence in the general population varies from 1 to 4%, but in Brazil there is a lack of statistics. It is a complex disease with a wide spectrum of phenotypes. Pathological anatomy (PA) of the polyp is the reference method indicated for analyzing eosinophils. Nasal lavage cytology (LN) is a less widespread and less invasive test. Objectives: The main objective is to compare eosinophilia between the AP and LN methods in CRSwNP. Secondarily, evaluate the association of tissue eosinophilia with disease control; the severity of asthma; awareness of aeroallergens; endoscopic and tomographic findings; Total IgE and peripheral eosinophilia. Methods: cross-sectional study, which compared: the degree of agreement for detecting eosinophilia between AP and LN methods; and peripheral eosinophilia with tissue, in relation to the diagnosis of the Type 2 endotype in CRSwNP. In addition to analyzing clinical and Conclusions: epidemiological factors associated with tissue eosinophilia, the following were performed: SNOT22, asthma control assessment, Prick test, laboratory tests, nasal endoscopy and tomography. Result: 30 patients were captured. The average age was 52 years. All were characterized as eosinophilic by one of three methods: peripheral eosinophilia, AP or total IgE. Eosinophils were altered by: 70% in the periphery if the cutoff point was above 250 cells/mm 3 and 83.3% when considering above 150 cells/mm 3; 86.6% in AP and 30% in LN. Total IgE was altered by 63.3%. Eosinophilic LN is statistically associated with elevated tissue eosinophilia. Peripheral eosinophilia with a cutoff point of 150 cells/mm 3 was the test that came closest to the gold standard, AP. LN is a non-invasive and easy to perform method, however it has been shown to be inferior to peripheral eosinophilia and total IgE.pt_BR

dc.description.resumoA Rinossinusite Crônica com Pólipos Nasais (RSCcPN) decorre da degeneração da mucosa nasossinusal. A prevalência na população geral varia de 1 a 4%, mas no Brasil carece de estatísticas. Ela é uma doença complexa com grande espectro de fenótipos. Anatomia patológica (AP) do pólipo é o método de referência indicado para análise de eosinófilos. A citologia do lavado nasal (LN) é um exame pouco difundido e menos invasivo. Objetivos: O objetivo principal é comparar a eosinofilia entre os métodos AP e LN na RSCcPN. Secundariamente, avaliar a associação da eosinofilia tecidual com controle de doença; à gravidade da asma; sensibilização para aeroalérgenos; achados endoscópicos e tomográficos; IgE total e eosinofilia periférica. Métodos: estudo transversal, que comparou: o grau de concordância para detecção de eosinofilia entre os métodos de AP e o LN; e a eosinofilia periférica com a tecidual, em relação ao diagnóstico do endótipo Tipo 2 na RSCcPN. Além da análise de fatores clínicos e epidemiológicos associados à eosinofilia tecidual, foi realizado: SNOT22, avaliação de controle da asma, Prick test, exames laboratoriais, endoscopia nasal e tomografia. Resultado: Captados 30 pacientes. A média de idade foi de 52 anos. Todos foram caracterizados como eosinofílicos por algum dos três métodos: eosinofilia periférica, AP ou IgE total. Os eosinófilos tiveram alterados em: 70% na periferia se ponto de corte acima de 250 células/mm 3e 83,3% ao considerar acima de 150 células/mm 3; 86,6% na AP e 30% no LN. O IgE total esteve alterada em 63,3%. O LN eosinofílico se associa estatisticamente com a eosinofilia tecidual elevada. Conclusões: A eosinofilia periférica com ponto de corte de 150 células/mm 3 foi o exame que mais se aproximou ao padrão-ouro, a AP. O LN é um método não invasivo e de fácil realização, entretanto se mostrou inferior à eosinofilia periférica e ao IgE total.pt_BR
